Your First Clinic Visit- Q&A

After setting up an appointment with one of our doctors we ask you please arrive 15 minutes early for your appointment so we can process your paperwork.

What do I bring?

Please bring your MSP card with you and all information you have on previous doctor's names and addresses should you wish to transfer your medical records.

Please bring any other information you may have that pertains to your health management such as current perscriptions, a list of any additional herb or vitamin supplements you are taking, any pending test results you may be waiting for, and a list of innoculations if you have those records.

What if I have questions?

It is a good idea to write down the questions you have for your new doctor, as we all know we can become nervous or anxious and may forget to ask those important questions.

What else can I do to prepare for my appointment?

If you wish you may print out and complete an online copy of our patient information form.
When you have filled in all of the appropriate information simply print the form and bring it with you to your first appointment.

Transfer of Records

If you need to have your clinical records transferred to or from our clinic you can download this form.

Please understand that this service is not recognized by MSP as a "medically required service" and therefore there may be a charge for this service of up to $40 (BCMA fee guide item A00093). We have arranged a secure website to enable electronic transfer of your file if this is more convenient for your new Family Doctor. Please ask our staff for details.
